Get your regular A1C blood test and receive a $20 gift card!*
You may know, getting your A1C test helps to manage your diabetes. This test looks at your blood sugar level over the past 3 months. It helps your doctor see if your diabetes care plan is working (it’s good to do this at least once a year). It’s a blood test done at a doctor’s office/lab (different from testing your blood sugar at home).
A1C Test
- Completion of your A1C blood test – After taking the test, Fill out this form and you'll receive a $20 gift card! When you go to your checkup, talk with your doctor about getting your A1C blood test if you have not done so already this year.
*To qualify, participants must be an active member living with diabetes. The test must have been completed within the last year. Limited to one gift card (up to $20 value) per year.

Complete Your Preventive Screening and
Enter to Receive a $20 Gift Card
Completing your cervical cancer screening, also known as a Pap test or Pap smear, is one of the best things you can do to prevent cervical cancer. For most women, this will likely happen at your regular checkup. This screening is free* as part of your WHA health plan and a safe way to protect your health.
When you complete your cervical cancer screening (Pap test/Pap smear) – Fill out this form to receive a $20 gift card. When you have your yearly checkup, ask your doctor (or OB/GYN) about getting your pap test if you have not done so already. When you complete your test, we invite you to complete the entry form to claim your $20 gift card - it's just WHA's way to say "good for you" for taking care of your preventive health!
*To qualify, the exam must have been completed within the past year. Limited to one submission per year. Coverage for WHA services depends on your plan and eligibility at the time of service. Please see Official Rules, as only group and individual members are eligible; CalPERS, Covered California, and Medicare members are not eligible.

Free Blood Sugar Monitor Offer!
Available to WHA members with either Type 1 or Type 2 diabetes and who want to monitor blood sugar levels, Optum is offering blood sugar meters at no cost. Talk to your doctor or diabetes educator. They can help you select the meter that’s right for you. Optum has two models available.
Get a voucher to redeem locally. When WHA members request a Contour Next meter from Ascensia Diabetes Care fulfillment centers, they will now receive a voucher. Members can use the voucher to redeem a meter at no cost from their local in-network pharmacy counter, where they also obtain test strips and other diabetes supplies.
Vouchers are available through their call center by calling: 800-401-8440. Vouchers are available on the external Ascensia Diabetes Care website without requiring a login.

WHA members interested in getting fit and staying healthy can choose Active&Fit Direct, which allows them to work out at any fitness center in their network. The monthly cost is just $25 (plus tax where applicable) – that’s less than $1 a day! In addition, WHA has partnerships with other health clubs that are not part of the Active&Fit network. Gift Card Incentive
Please review the recommended immunizations (see shot schedule) that should be given at your baby’s wellness visits. These vaccines are free* as part of your WHA health plan, and a safe way to protect your child’s health.
As you complete immunizations at well-baby visits, complete the form to get your gift card.
*Coverage for WHA services depends on your plan and eligibility at the time of service.
Incentive Options
Incentives are open to children aged 0 – 24 months. Members can submit a subsequent form upon completion of immunizations at each well-baby visit (between July 2023 – July 2025).
WHA will notify you via email. By default, rewards are available as e-gift cards, which can be downloaded and used instantly.
Be Proactive and Stay with the Program
Participate in the Virta program for 60-days and enter for a chance to win a $50 gift card*
WHA partners with Virta Health for an innovative telehealth program that works to reverse diabetes (as well as lower your A1C, lose weight, and reduce/eliminate medications). We’re seeing positive results among members who stay on the program. So, we've added an incentive not just for joining the program, but for sticking with it to see significant results! For members enrolled for at least 60 days and stayed on the program’s eating plan and home testing recommendations, you can enter to win a reward.
Virta Program Incentive
- Completion of 60 days in the Virta Program – To reward enrolled members, fill out this form and you’ll be automatically entered for a drawing* to win a $50 gift card, just for staying with the program!
*To qualify, participants must be active members between 18-79 years of age and living with type 2 diabetes, participating/enrolled in Virta for at least 60 days, and following their eating plan and testing requirements. See official rules.
