Incentives Official Rules
For WHA Incentives: Limited to one submission per year.
All federal, state, and local laws and regulations apply. Void where prohibited by law. All taxes, fees, and surcharges on the prize are the sole responsibility of the winner. The winner will be notified by email or phone call. If the winner cannot be reached at the email address or phone number submitted with entry, the prize will be forfeited and an alternate winner will be selected. By participating in this incentive program, participants agree to be bound by these Official Rules and the decisions of WHA regarding administration of the incentive program are binding and final. Failure to comply with these Official Rules will result in disqualification.
This incentive program is open to WHA members who:
- Are California residents and currently live or work in one of the following counties: Alameda, Colusa, Contra Costa, El Dorado, Humboldt, Marin, Napa, Placer, Sacramento, San Francisco, Solano, Sonoma, or Yolo.
- Have successfully completed a qualified preventive exam (e.g. mammogram, DRE, or A1C, or other specified preventive test/activity noted in a WHA incentive program) within the past year.
- Are currently an active member, commercial and individual members only. CalPERS, Covered California, and Medicare members are not eligible. Please visit mywha.org/rewards to access your available rewards.
To enter, you must complete the entry form located on WHA's website. Incomplete submissions may not be processed.
Incentive recipients will be awarded an electronic gift card, selected by WHA, during the last week of each month from all eligible entries received within the preceding month.
WHA selected gift cards. WHA will notify you via email. Please allow up to 6 weeks for processing.
Participants agree to hold harmless WHA and its employees from any and all liability, claims, demands, or causes of action for personal injury and damages arising from participation in the sweepstakes and use of the prize.
Acceptance of the prize constitutes permission for WHA to use the winner's name, photograph, and likeness for purposes of advertising, trade, publicity, and promotional purposes without further compensation, in any and all media, without notice, review, or approval.
